If
you cannot type into the above forms, you may be viewing the file in
PDF/A mode. In Adobe click Edit > Preferences > Documents >
View Documents in PDF/A mode. Select "Never."
Payment
Each time a course record is submitted, it must be accompanied by
payment. We accept
payment by check, VISA, Mastercard, American Express or purchase order.
Certificates
Please allow us 10 business days to process completed course records
and issue certificates. Certificates will be mailed to the instructor
or authorized provider for the instructor's signature and then the
instructor or authorized provider must distribute the certificates to
the students.
